As long as humanity exists, so many people suffer from headaches. The scourge of the 20th century was migraine - another type of headache. How modern medicine can help in this case, says Alexander AVSHALUMOV, director of the Moscow Institute of Cybernetic Medicine.

Alexander Semenovich, what is the difference between just a headache and a migraine?

By their external manifestations, it is not so easy to distinguish between them. But the origin of pain is different, and, therefore, the methods of dealing with it are also different. With an ordinary headache, we all know that it is enough to take a vasodilator and the pain will go away. What is the mechanism of headache in migrainewhen a person feels acute throbbing pain, nausea, photophobia, etc.?

It turned out that at migraine there is an increased activity of the adrenal glands. They are responsible for adapting the body during stress and release adrenaline, which constricts blood vessels. If during a normal headache the adrenal glands release adrenaline once, then during a migraine-like attack they continue to work hyperactively, constantly producing this stress hormone for a long time.

A person's head is "splitting", the body spends all its energy on something to compensate for the incessant effect of adrenaline on the narrowed vessels. And if at this moment a person takes a vasodilator drug, it does not work: its therapeutic effect is not enough to suppress the constantly emitted adrenaline and stop vasoconstriction.

What are the causes of a migraine?

There may be several reasons. One of them is neuroses, single or permanent. For the development of neuroses in our body, the neuroendocrine system (including the adrenal glands and the thyroid gland), which goes into a mode of increased activity, is primarily responsible. The constant production of stress hormones, including adrenaline, leads to vasoconstriction, and not only once, but permanently.

How does modern medicine investigate the work of the adrenal glands?

Accurately assessing the work of the adrenal glands is a very laborious task today. World medicine knows only a few such methods, all of them are quite complex and uninformative. However, we managed to find a relatively simple method for determining the state of the adrenal glands. We have developed a double dynamic stress hormone test. In short, its essence is as follows: a person takes a blood test for stress hormones (adrenaline, norepinephrine, dopamine). After that, he receives some stressful influence (for example, he is asked to listen to exciting, disturbing music), and then he re-donates blood for stress hormones.

Thus, we get the opportunity to compare the level of these hormones. before and after procedures and assess the adrenal gland's response to stress. By the difference in the levels of stress hormones, you can tell how much the adrenal glands are disrupted.

It is known that women suffer from headaches much more often than men. What is the reason for this?

Indeed, according to statistics, women suffer from migraines 10 times more often than men. The reason for this is an imbalance in some hormones. This is due to the fact that men have only one main sex hormone, testosterone, while women have two - estrogen and progesterone. Cyclic work in the female body of these antagonistic hormones affects a woman's mood, her performance, well-being, fertility. And the role of the adrenal glands cannot be underestimated here either. After all, they produce female sex hormones, and disruption of their work leads to hormonal imbalance, which provokes a headache. Many women can confirm: very often they have headaches in the premenstrual period.

Another reason for the occurrence migraine there may be infections. Our brain has a so-called blood-brain barrier that protects it from the penetration of various substances and infections. Unfortunately, this protection does not work against neuroinfections. I'll give you a simple example. One of our patients with a headache was diagnosed with toxoplasmosis, a disease that is carried by our pets, cats. As it turned out, this woman really had a striped beauty in her house.

If we assess the prevalence of all these reasons, then neuroinfections are quite rare, approximately once in 300 people, hormonal imbalance (especially in women) we note in every third case, and adrenal hyperfunction is detected in almost every patient of ours.

But how do you get rid of the pain when conventional head medications do not help?

If we believe that a seizure migraine - this is not an ordinary headache, but, first of all, hyperfunction of the adrenal glands, which constantly produce adrenaline, literally pressing the vessels, then we must first stabilize the function of the adrenal glands and thyroid gland. Only after that vasodilating drugs can provide real help, improve microcirculation, increase the supply of oxygen to the brain, glucose, and other substances. Therefore, it is necessary to change the very approach to migraine treatment.

What does modern pharmacology offer for the treatment of migraines?

Pharmacologists offer a seemingly paradoxical solution: stop seizures migraine vasoconstrictor drugs. The logic here is as follows: if the vessels are constricted and they are further constricted, a "string" effect arises, when a reverse signal goes through the superconstricted vessels to the adrenal glands and they slightly reduce their activity, the patient's condition stabilizes and relative relief occurs. But, unfortunately, the drugs that are currently offered for the treatment of migraines do not always help and work only for a while, so the situation is far from stabilizing.

It is this algorithm of actions that allows us to achieve success. If relapses occur, then after a considerable time, which is already a great progress in treatment migraine.
